Bought a book a couple of years ago that may fit the bill.

Slide Guitar by Fast Forward by Rikky Rooksby.
Got it from local guitar store. Nice and simple and straight forward info.
    Hope this helps.

Progressive slide guitar is pretty good, I have several of their series and find them great.
for sale, MXR Zakk Wylde overdrive. PM if you're interested.

Arlen Roth's book? It covers a variety of tunings used.

Future ( people behind Guitarist, Total Guitar and Guitar Techniques) did a Blues Slide magazine special. It is at a sale price of £3.99(delivered) on their website.

Ahh - looks like it came with a dvd rather than a cd but its a pretty cheap starting place none the less.
